Build marketing forms with consent

Create a form, add consent boxes and progressive fields, publish it and embed it on your website.

For: Marketing and sales managers

Steps

  1. 1Go to MetriReach → Forms and select New form.
  2. 2Name the form, check its web address, and write the title and intro in English, Bahasa Melayu and 中文 using the language buttons.
  3. 3Add fields and drag them into order (or use the arrow buttons). Map fields to the contact's name, email, phone, company or job title; a marketing form needs an email or phone field.
  4. 4Tick Progressive on fields you only need once: visitors whose contact already has that detail don't see the field again.
  5. 5Select Add consent box for the privacy notice acknowledgement and for marketing by email, WhatsApp or SMS. Boxes are never ticked in advance; only the notice box can be required.
  6. 6Under After submit, choose whether to also create a sales lead and its team, the thank-you message or a page to go to.
  7. 7Select Publish, then copy the embed script (or the iframe code) into your website, or share the link.

Good to know

  • Each submission creates or updates the contact (matched by email, then phone) and only fills empty details; it never overwrites what your team entered.
  • Consent is recorded with the source and the privacy notice version. A form can't undo someone's earlier unsubscribe.
  • Every submission can start an automation or workflow with the trigger MetriReach form submitted.
